Haemoptysis

Question
Our 13 year old son had three bleeding from June 2010 to January 2011. He underwent three embolizations ... What can be done to prevent recurrences?
Answer
Hello,
Pulmonary bleeding (or haemoptysis) in cystic fibrosis is favored by pulmonary exacerbation due to infection in the lungs. The best way to prevent further haemoptysis is to treat intensively chronic lung infection. This includes regular treatment with chest physiotherapy and antibiotics via aerosol, oral or intravenous infusions depending on the type of bacteria present in the bronchi. An anti inflammatory treatment with drugs such as azithromycin may be of interest. It is also indicated to check that there is no bleeding disorder, such as a vitamin K deficiency, which can then be corrected. Treatment with aspirin must be avoided as it may promote bleeding. Some centres use treatment with tranexamic acid (Dicynon) but there is no consensus on this. The most important thing is to optimize the prevention of severe respiratory infection. However, we cannot absolutely guarantee that haemoptysis may not recur.
Best wishes.
