I would like to know if there is a threeshold FEV1/FVC necessary before pregnancy. What are the other criteria: BMI, hemoptysis, other...
Answer
Yes of course, FEV1 is an important criteria to consider before pregnancy. There is not exactly one minimal threshold value. It is usually considered that pregnancy is possible for women with an FEV1 greater than 50% of excepted value without significant risk for respiratory status, but it is important to consider other clinical complications such as diabetes mellitus or liver cirrhosis. When FEV1 is below 30% or in case of hypoxia, pregnancy is considered to be a high risk for women. When FEV1 is between 30 and 50% it is important to consider the stability of respiratory function, the nutritional status, the frequency of exacerbations and the resistance of microbial strains that infected bronchial secretions, ...
In all the cases it is important to consider pregnancy in a stable state. Active hemoptysis is therefore a contraindication for pregnancy. Nutrional status and diabetes mellitus are also very important to consider.
In all cases, a genetic counselling is necessary.
